Definitions are provided at the end of the printable version of the survey and in the online survey by clicking on the linked words. The definitions will open up in a seperate pop-up window. Please make sure to set your browser to temporarily allow pop-ups from www.ncslisurvey.com to see the definitions.

Save your progress during the survey by using the 'Next Section' and 'Previous Section' buttons at the bottom of each page to navigate through the survey. Please do not use your browser's Back or Forward buttons to move between sections as this will cause you to lose the data you have previously inputted.

To quit a survey before completion, press the Quit button at the bottom of the page. To keep surveys anonymous, incomplete surveys cannot be saved. By quitting you lose all data and you must start a new survey.
